The first step towards a beautiful, healthy smile is to schedule an appointment. Please contact our office by phone or complete an online appointment request.
If you are a new patient to our office, please visit our patient forms page to access our online new patient b forms that will need to be filled out when you arrive at our office. Filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Thank you and please contact our office if you have any questions at all.
Our main goal is for you to obtain the highest quality of dental care by providing you the service that is based upon a professional, courteous and confidential working relationship between you and our office.
At your reserved appointment, we are able to give you a cost estimate based on your advised treatment. If you have insurance, we will verify your benefits and give you a cost estimate based on those benefits.
As with any estimate, if for any reason it is incorrect, you are responsible for your balance.
We require that all patients, with and without insurance, pay for their treatment at the time of service.
As a courtesy, we contact your dental insurance provider to check your benefits and submit claims on your behalf. We are not responsible for and can make no guarantees as to the coverage your dental insurance will provide. As the patient, it is ultimately your responsibility to know your dental insurance and benefits and you are responsible for any balance that is not paid by dental insurance. We require that all patients, with or without insurance, pay for their treatment at the time of service.

In the event of a returned check we reserve the right to require you pay in cash in the future.
